To print the form(s) correctly, you must first select either the Spay / Neuter, Membership, Foster Care or Adoption Contract tab(s) above then click on File then click on Print Preview. At the top middle of the Print Preview page, change "As laid out on screen" to "Only the selected frame" by using the blue down arrow button just to the right of the small window. Then click on the print icon located at the top left of the screen to print the form(s).
After printing the form(s), please fill out the information requested and mail it / them to:
